Add documentation for vc-ignore and vc-dir-ignore.

* doc/emacs/emacs.texi (Top): Add menu entry.

* doc/emacs/maintaining.texi (VC Ignore): New node.  Document vc-ignore.
(VC Directory Commands): Add vc-dir-ignore.
